Just click on the words "submit a request" at the upper right of the browser window. You can also view your past submissions and see all of our case reports for your hospital all in one place. If you log in and don't see your cases, it is likely that you are using a different email address than what you used to fill out our case request form. Just send an email to [email protected] and let him know what email address you used to log in.
